Qualification Criteria
Staff Nurse
Diploma in General Nursing and Midwifery or B. Sc. Nursing from any Recognized Institution Approved by Nursing Council of State/Government of India.
Registration from up Nurses and Midwives Council at the time of submission of online application.
ANM
Certified Diploma in Auxiliary Nursing and Midwife by any Recognized Institute approved by Nursing Council of the State/Government of India.
Registration from up Nurses and Midwives Council at the time of submission of online application.
Pharmacist-Allopathic
Intermediate with Diploma in Pharmacy/Degree in Pharmacy.
Registration from Uttar Pradesh Pharmacy Council at the time of submission of online application.
Lab Technician
Degree in Medical Laboratory Technology or Intermediate (10+2) in Science with Diploma in Medical Laboratory Technology.
The Degree/Diploma is being from a Recognized University/ Institute by Central Government or State Government. Registration from UP State Medical Faculty.
Age Limit As on 12.12.2022
Minimum Age Limit : 18 years
Maximum Age Limit : 40 years
Age Relaxation : For SC/ST – 5 Years , For OBC – 3 Years

1. Go to official website and click on ‘Updates – View All – Online application invited for vacant contractual Positions……’.
2. Then click on PROCEED TO REGISTER.
3. Fill Personal details, Program name, Post Name, Your Name, Mobile No., Email Id and then submit.
4. Then Click here to fill Application Form.
5. Then select examination centres.
6. Fill up Personal details, Address details, SSC & HSC Examination Details, Additional Qualification details.
7. Upload your photo, signature, D.O.B. certificate, Qualification certificates and other required certificates in proper format.
8. Finally submit the application form and keep the printout of the application form for future use. And do not send the hard copy of the application form & other documents to the UP NHM office.
